Amendment
PHS Policy on Humane Care and Use of Laboratory Animals (Policy) (IV.C.1.) and Animal Welfare Regulations (9 CFR 2.31 (d) (1) (i)- (iv)) requires IACUC review and approval of changes to animal activities.
Study staff are responsible for promptly reporting changes to approved research prior to changes being implemented.
How do I start an amendment Form?
Login to iRISUnder the table, All Studies, find your approved study.
On the protocol assistant dashboard, select Forms on a study in Approved Status.
Select IACUC Amendment Form to open the online amendment submission form.
Or open the protocol dashboard, click amendment. Click add new form.
