How to Get Started
What can I expect in my first consultation?
The COD will gather information about your concerns and determine what service(s) best meet your needs and will then assist you in connecting with the most appropriate services and resources. If you are in crisis, the COD will collaborate with you in creating a plan to cope with the crisis.
How do I check in for my phone or Zoom appointment?
PLEASE LOG INTO THE PATIENT PORTAL USING ANY MOBILE DEVICE 15 MINUTES BEFORE YOUR SCHEDULED APPOINTMENT TO COMPLETE THE FOLLOWING TASKS:
Step 1: Click on "Check-in for an Appointment"
Step 2: Verify your phone number/mobile carrier/address are correct. You may be prompted to “Capture a Photo”- THIS IS NOT NECESSARY- and you can select “Continue” to skip this step.
Step 3: Complete the THREE required UCC consent forms. Completing the UCC TELECOUNSELING 20 FORM accurately is REQUIRED when you check in for your appointment.
Your FIRST CLIENT SUPPORT PERSON must be:
- Someone who is located within the same city where you CURRENTLY live.
- Someone who knows you receive services at the UCC and
- can respond to your location in an emergency situation.
Your SECOND CLIENT SUPPORT PERSON/PROFESSIONAL CONTACT may be:
- Someone located anywhere in the United States
- Someone who you are comfortable with being contacted to assist you in an emergency situation.
- Their name
- Their relationship to you
- Their phone number
You MUST also list the name and contact info for a LOCAL HOSPITAL which offers EMERGENCY MENTAL HEALTH CARE close to where you live. If you do not live in Corpus Christi, you will need to look up the name, address, and phone number of the closest hospital.

Step 4: Complete the informational questionnaire about your situation & reasons for seeking services.
Step 5: Complete the pre-visit CCAPS survey based ONLY on how you have been feeling over the PAST TWO WEEKS.
Step 6: Once you finish the pre-session forms, click "Join Meeting" to be directed to the Zoom videoconference room, and wait for the counselor to join you. If you requested a phone call, don’t click “Join Meeting,” The COD counselor is notified when you complete your forms and will call your shortly.
