Reportable Event
For safety reasons, all accidents should be reported, regardless of severity. See TAMU-CC, Environmental Health and Safety, Laboratory, Emergency Procedures.
Timing: Investigators must submit a reportable event as soon as possible. Once emergency response has an incident report, the event must be reported to the IBC within five (5) business days.
To submit a reportable event
On the study assistant dashboard, find the protocol in the All Studies table.
Under Actions, select Forms.
Select TAMU-CC IBC Reportable Event Form to open the online reportable event submission form.
