How do I start an annual check-in or continuing review form?

Sixty days before your expiration you will receive a notice that an Annual Check-in or Continuing Review is due. You will also have an outstanding tasks on your Study Assistant Dashboard.

Login into iRIS. On the study assistant dashboard, select Forms on a study in Approved Status.

Select “Annual Check-In” or “IRB Continuing Review Form” to open the online submission form.
