Disability Related Housing Accommodations

Required Documentation for Housing Accommodation Requests

ALL items are required for approval.

  • Provider & Relationship: Your documentation or letter (PDF or photocopy, no Word documents) must come from a doctor or medical professional with whom you have an established and ongoing therapeutic relationship. This relationship must be at least 30 days in duration at the time the letter is issued, and should involve more than a single evaluation solely for the purpose of this accommodation request.

  • Official Letterhead: The letter must be on the provider's official letterhead.

  • Recency of Documentation: The documentation must be dated within the last year and should not be expired.

  • Connection to Disability: The letter needs a statement explaining how a disability, which substantially limits one or more major life activities (as defined by the Americans with Disabilities Act), connects to the need for your housing accommodation request. While specific medical history isn't required, there should be an indication that the disability affects your daily functioning.

  • Explanation of Need: Your documentation must clearly explain how the requested accommodation is necessary to alleviate specific symptoms or effects related to your disability. Specifically, it should detail how the request directly contributes to your well-being and enhances your ability to function in a residential setting.

  • Credentials and Signature: The documentation must include the provider's name, license number, and contact information. The provider must be licensed and in good standing with their respective licensing board, and the letter must be signed by the provider with a "wet" signature (physically signed).

Please note: Beware of websites that offer immediate ESA letters or certifications for a fee without a thorough evaluation or an established relationship with a licensed professional. Such letters may not meet our documentation standards.

We may contact your healthcare provider to verify the information provided or to request further clarification, provided we have your signed consent to do so. 

Important Notice Regarding Documentation Integrity: 

Providing false or fraudulent documentation for your housing accommodation request is strictly prohibited. Such actions may lead to an incomplete application and disciplinary action per the Student Code of Conduct.
