REQUIREMENTS INVOLVING PARENTS/GUARDIANS |

1.1 |
Parents/guardians of minors participating in co-curricular
services on campus must give written permission for their minor to
participate. |

1.2 |
Parents/guardians are responsible
for remaining with their minor on campus during these services, as
appropriate. When not appropriate for parents/guardians to remain
in the same room with their minor, parents/guardians should remain
in the near vicinity to assist with any issues that may arise. |

Parents/guardians will supply the
University with emergency medical information and a Waiver of Liability/Consent
to Treat before their minor may receive co-curricular services on
campus. |

PROCEDURES INVOLVING COOPERATING AGENCIES
For co-curricular activities that occur off-campus, the University
and the cooperating agency shall have a memorandum of understanding
that outlines the procedures students must follow to deliver services
through the cooperating agency.
